2022 Teen Summer Volunteer Application

  1. Submit an application for the 2022 Teen Summer Volunteer Program between May 11 - June 4, 2022.

    Use this form to apply to be a teen summer volunteer at the library. The volunteer program runs between June 13 and July 29, 2022.

    Note: Parents or caregivers needing to register a child aged 3 - 11 for the Book Buddy program must use this form instead:

    Monitor Library Computer Lab and assist customers with minor computer questions. Assignments are Monday - Friday from 10:00 AM to 4:00 PM. You must be available for a minimum of 2 hours one day a week, although you may request additional hours/days. You must attend a mandatory Teen Techie orientation and training on Tuesday, June 14, 2022 at 12 PM. Schedules will be set at this time. No exceptions. Are you available at this time?

  13. Program Terms of Participation*

    • If I am selected as a Teen Volunteer, I will abide by all the rules of the DeSoto Public Library and the Teen Summer Volunteer program. 
    • I will be available a minimum of 6 weeks. 
    • I will attend the volunteer orientation on Saturday, June 11 at 12 PM (noon). 
    • I understand that I am offering to volunteer free of charge. 
    • I also understand that if I do not abide by all the rules of the DeSoto Public Library and the Teen Volunteer Summer Program, that I will be dismissed from the program immediately.

  15. Medical Emergencies Involving Minors:
  16. In the event that a parent or legal guardian cannot be reached in a medical emergency, the DeSoto Public Library is authorized to arrange for emergency medical treatment, the cost of which will be the sole responsibility of the parent or legal guardian.
  17. Parent/Legal Guardian Permission:*
  18. I authorize the DeSoto Public Library to arrange emergency medical treatment, in case of an emergency. I understand that my son/daughter will not receive monetary compensation or be insured by the Library.

  19. The Volunteer Coordinator will select applicants and arrange the interview.
  20. Selected volunteers must abide by the TSVP attendance policy, dress code and expected work conduct and will be required to interview and submit a background check. Arrangements for the final work schedule will be made by the Teen Volunteer Coordinator.
  21. There are only a limited number of volunteer hours and positions available for the summer! Therefore, all teens that apply are not guaranteed placement in the program.
